Waitlist Camper Registration Form

We are at capacity for campers right now. Please fill out the waitlist form, and we will do our best to place your camper!

Camper Registration - complete this form for EACH camper

    Camper Information:  Please provide accurate camper information including the name, birthday, age and number of years as a Sammamish Day Camp (SCD) camper. 


    Grade/Size Information: Please ensure the grade information is correct.  If you child is registered in the INCORRECT grade, she will only be admitted if there is space available in the correct grade.
    ​Also note that shirts are worn over whatever your child wears to camp, and Youth Small shirts are VERY small.

    Registered Girl Scout:
     Girl Scouts of Western Washington requires that all camp attendees are currently registered members of Girl Scouts USA.  If you are not currently registered, please visit the Girl Scouts of Western Washington's website to complete registration.

    ​ Please make sure you complete the registration by May 15th!
    Registered Girl Scouts provide troop information.

    Camp Buddy:  Your child has the option to list a camp buddy. Note the friend must also list your child as their "buddy". While we want to make sure that every camper has a friend at camp, we create pairs (not trios). Buddy placement is not guaranteed though we strive to organize camp so that it is the best experience for everyone. Even if your daughter doesn't know anyone at camp, we promise she will leave with great friends & memories!

    Siblings:  List names of sisters that are registered (or are registering to be a Camper, a PAT or PA.  Leave blank if not applicable. 

    If parent (or other family member) has registered to volunteer, please list the adult volunteer name.  Leave blank if parent (or other family member) is not volunteering.

    Special Considerations:  Are there any special needs we should consider when placing your child in a camp unit? (e.g. severe allergies, dietary restrictions,  or other health or behavioral related concerns).

    Medical Conditions:  Are there any medical conditions that may affect your child's camp experience? Please list any and all issues so that we may be fully prepared to give your child a great camp experience.
    Allergies: Does your child have any allergies to food, medications, or the environment? Please list these here! They will also need to be listed on the required health forms.

    Section B. Camper Parent Information



    ​Authorized Adults for Pick UP:  Please list ALL adults (including BOTH parents) authorized to sign your child out of camp. NOTE that they will have to show picture ID when picking up and we will NOT release campers to anyone not on your list.

    Section C. Cookie Dough or Financial Assistance options (Applies to current Girl Scouts only, not Tagalong and Boys)
    FINANCIAL ASSISTANCE Girl Scouts of Western Washington provides financial assistance as needed to support members to attend programs. All financial assistance applications must be received in our DuPont office no later than 30 days before the first day of camp. Forms and instructions are available at https://www.girlscoutsww.org/get-involved/Join/Pages/Financial-Assistance. Questions about financial assistance should be directed to your regional GSWW office or the Financial Assistance Coordinator at 1-800-541-9852.

Submit Camper Registration
Once you successfully submit the form, you will see a thank you screen.
​Please give us until April 16th to begin sending the confirmation and next step emails.

​If you have more than one camper to register, please go back and register the next camper now.
Each camper must be registered individually.
